An Illinois accident or crash report is officially titled as Illinois State Traffic Report SR 1050 ’. It is used by law enforcement to record the details relating to an accident involving any type of vehicle. The details in the report are recorded by the officer who attended the scene of the accident. If a police officer did attend your accident, you must file a motorist report with the local police department, sheriff’s office, or Illinois State Police as soon as possible. You can file this report online through the Illinois Department of Transportation. Each driver involved in an Illinois traffic crash must file a crash report if the crash caused a death, bodily injury, or more than $1,500 of property damage when all drivers are insured. 2. Order Online Through Illinois Department of Transportation Motorist Report Portal

To request your crash report online from the Department of Transportation, you will need to provide information about the accident and specific driver or vehicle involved in the crash. You will also need to provide your name, mailing address, email address, and the applicable fee of $5 per report. The online portal can be found here.

3. Other Data Sites like LexisNexis or CrashDocs

These can be a good reliable source of data. However the sites can be difficult to use and do not have all the Police Agencies in Illinois. (sometimes less than 50%)

4. Illinois Crash Report by Mail

To request a copy of your crash report in writing, by mail, compose a letter listing the same details that are requested in the online form, including:

  • A driver's first and last name
  • The date of the crash
  • The location of the crash, including the Interstate and County

The Agency Crash Report Number (located in the upper-middle portion of the motorist report) Include a check or money order in the amount of $5.00 for a traffic crash report made payable to “Illinois State Police.” Do not send cash.
