The base cost for requesting an accident report in North Dakota is $2, which will provide you with the officer’s report alone. For an additional $5 you can include the officer’s opinion as well. To pay these fees you will include a check or money order along with your request.

Request a North Dakota Collision Report Via Mail

To request a crash report in North Dakota via mail (the only option available at this time), you will need to fill out the Request for Crash Report Information form (SFN-4901).

When you fill out this form, you must fill in all of the fields as accurately as possible to ensure that there is enough information available to pull up the associated accident report. The first section of the form is needed for the Officer’s Report, which has a fee of $2. If you are not looking for an Officer’s Opinion, you can ignore the second half.

However, if you are looking for the Officer’s Opinion, you will need to pay an additional $5 ($7 total) and fill out the remainder of the form. This second part of the form requires you to confirm your identity to ensure you’re allowed to request a crash report. To request the Officer’s Opinion, you either need to be:

  • A party to the crash
  • A party’s legal representative
  • An insurer to a party of the crash

When you are requesting an Officer’s Opinion, you will also need to provide a valid reason for requesting the Officer’s Opinion.
